Can You Fail an HGV Medical? Common Reasons Explained

Before hitting the road in a heavy goods vehicle, every driver must pass a medical exam to prove they’re fit to drive safely. But can you actually fail an HGV medical? The answer is yes and understanding why can help you avoid delays in your training or career. In this blog, we’ll break down the most common reasons drivers fail the medical and how to get prepared.

What Is The HGV Medical?

The D4 form is used to evaluate a driver’s physical and mental health to determine if they are fit enough to drive a large vehicle safely. This medical examination is a mandatory requirement, completed by a doctor, to assess a driver’s fitness to drive large vehicles, ensuring they meet the requirements set by the DVLA (Driver and Vehicle Licensing Agency).

All commercial drivers need an HGV medical assessment before they can get on the road, then after that it may need renewing; D4 medical assessments are required to be renewed every five years for drivers over the age of 45 but under the age of 65, and every year for drivers over the age of 65.

What Happens If You Fail?

Failing doesn’t always mean the end, often issues can be resolved with medical support or proper documentation.

Common Reasons For Failing

How To Prepare And Pass

One of the best ways to avoid surprises on the day of your medical, is to get a general health check-up with your GP before attending your official D4 HGV medical. This gives you a chance to address any potential issues, like high blood pressure or vision concerns, before they’re flagged during your assessment. If you wear prescription glasses or contact lenses, make sure to bring them to your appointment, as your eyesight will be tested to ensure it meets DVLA standards for commercial driving. Most importantly, be honest with the doctor conducting the medical. If you have an existing condition, don’t try to hide it. In many cases, having a medical issue doesn’t mean automatic failure, as long as it's well-managed and documented. Getting the right treatment or specialist support early can make all the difference in passing and getting on the road confidently.

What If You Fail?

Failing your HGV medical isn’t always the end of the road. In many cases, you’ll have the option to retake the medical after addressing the issue, especially if it’s something treatable like high blood pressure or vision problems. The DVLA may also refer you to a specialist for further assessment if they need more detailed information about a specific condition. During this time, they’ll review all medical reports and test results before making a final decision about your fitness to drive. If your health improves or you meet the necessary standards after treatment, you can reapply for your licence and get back on track.
